1. AUTHORIZED ACCESS ONLY. This portal ("ClaimsBloom360 Portal") is a proprietary, password-protected system owned and operated by GreenThumb Revenue LLC ("GTR"), a revenue cycle management company operating as a HIPAA Business Associate on behalf of its contracted healthcare provider clients ("Covered Entities"). Access is strictly limited to individuals expressly authorized by GTR or a contracted Covered Entity. Unauthorized access is strictly prohibited and may result in civil and criminal liability under applicable federal and state law.

2. CONFIDENTIAL AND PROTECTED HEALTH INFORMATION. This portal contains confidential business information and Protected Health Information ("PHI") as defined under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), the Health Information Technology for Economic and Clinical Health Act ("HITECH"), and their implementing regulations (45 C.F.R. Parts 160 and 164). All PHI, financial data, credentialing records, patient billing data, insurance claims information, Explanation of Benefits ("EOB") data, payer contract information, and any other content accessible through this portal is strictly confidential. Unauthorized use, access, disclosure, copying, reproduction, distribution, transmission, or modification of any such information is strictly prohibited and may constitute a federal criminal offense punishable by fines and imprisonment.

3. BUSINESS ASSOCIATE RELATIONSHIP. GreenThumb Revenue LLC operates as a Business Associate ("BA") as defined under HIPAA and maintains executed Business Associate Agreements ("BAAs") with all contracted Covered Entities. GTR implements administrative, physical, and technical safeguards as required under the HIPAA Security Rule (45 C.F.R. Part 164, Subpart C) to protect the confidentiality, integrity, and availability of electronic PHI ("ePHI") processed through this portal.

4. MONITORING; NO EXPECTATION OF PRIVACY. By accessing this portal, you expressly acknowledge and consent that: (a) all activity, access, and transactions within this portal may be monitored, recorded, and logged by GTR for security, compliance, and audit purposes; (b) you have no expectation of privacy with respect to any activity conducted on or through this portal; (c) any evidence of unauthorized or unlawful use may be provided to law enforcement; and (d) GTR reserves the right to terminate your access at any time without prior notice.

5. PERMITTED USE; RESTRICTIONS. Authorized users may access this portal solely for legitimate business purposes directly related to revenue cycle management services provided by GTR. You agree not to: (a) share your login credentials with any third party; (b) access or transmit any data beyond what is necessary for your authorized role; (c) use automated tools, bots, or scrapers to extract data; (d) attempt to circumvent any security controls; or (e) use any information obtained through this portal for any purpose other than its intended business use.

6. ELECTRONIC COMMUNICATIONS; SMS CONSENT. By accessing this portal and providing contact information, you consent to receive electronic communications from GreenThumb Revenue LLC, including email notifications, portal alerts, and claim status updates. If you have opted in to SMS communications, you consent to receive text messages from GTR at the mobile number provided. Standard message and data rates may apply. 7. DISCLAIMER OF WARRANTIES; LIMITATION OF LIABILITY. This portal and all content herein are provided "as is" and "as available" without warranty of any kind. GreenThumb Revenue LLC expressly disclaims all warranties, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. In no event shall GTR, its officers, directors, employees, or agents be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of this portal.

8. NO LEGAL; MEDICAL; OR FINANCIAL ADVICE. Nothing contained in this portal constitutes legal, medical, financial, or regulatory advice. GreenThumb Revenue LLC is a revenue cycle management company and is not a law firm, medical practice, or financial advisory firm. Billing estimates, claim projections, and reimbursement calculations are for operational and informational purposes only and should not be relied upon as guarantees of payment or compliance determinations.

9. BREACH REPORTING. If you suspect or become aware of any unauthorized access, data breach, or security incident involving this portal or any PHI accessed through it, you are required to report such incident immediately to GreenThumb Revenue LLC at Compliance@gtrcm.com or (802) 473-1501. GTR will investigate and, where required, notify affected parties and regulatory authorities in accordance with the HIPAA Breach Notification Rule (45 C.F.R. Part 164, Subpart D) and applicable state breach notification laws.

10. GOVERNING LAW; JURISDICTION. Your use of this portal is governed by the laws of the State of Vermont, without regard to its conflict of law provisions. Any disputes arising from or relating to this portal shall be subject to the exclusive jurisdiction of the state and federal courts located in Vermont.

11. CHANGES TO TERMS. GreenThumb Revenue LLC reserves the right to update or modify these terms at any time without prior notice. Continued use of this portal following any such changes constitutes your acceptance of the revised terms. Last updated: 2026.
